What is the Rotator Cuff Repair Protocol?

To fully recover and get back to your normal activities, you’ll need to follow a specific plan called the rotator cuff repair protocol.

What is the Rotator Cuff Repair Protocol?
The rotator cuff repair protocol is a detailed plan that guides your recovery after surgery. It includes specific steps and timelines to help your shoulder heal properly. The goal is to reduce pain, restore strength, and get your shoulder moving again.
The protocol is divided into four phases, and each phase has its own focus. Your doctor, along with your physical therapist, will design the plan for your specific needs. However, here’s a general idea of what to expect:
Phase 1–Protecting the Shoulder (0-6 Weeks)
Right after surgery, your shoulder will need time to heal. During the first six weeks, the focus is on protecting the repair and managing pain. You’ll likely wear a sling to keep your shoulder still and avoid any movements that could strain the healing tendons.
Your doctor may recommend gentle exercises to keep your hand and wrist moving, but you’ll need to avoid lifting, pushing, or pulling anything with the affected arm.
Ice and medication can help with pain and swelling. The key during this phase is to be patient and let your body heal.
Phase 2–Restoring Motion (6-12 Weeks)
Once the initial healing is complete, the next phase focuses on getting your shoulder moving again. You’ll start physical therapy to improve flexibility and range of motion. Your doctor and PT will guide you through gentle stretches and exercises to help your shoulder regain its mobility.
At this stage, you’ll still need to avoid heavy lifting or sudden movements, but you can gradually start using your arm for light activities. The goal during this phase is to restore movement without putting too much stress on the healing tendons.
Phase 3–Building Strength (12-24 Weeks)
By this point, your shoulder should be moving better, and the focus will shift to building strength. You’ll work with your PT on exercises that target the muscles around your shoulder. These might include resistance training, light weights, or resistance bands.
It’s important to follow your doctor’s instructions carefully during this phase. Overdoing it can slow down your recovery, so take it slow and steady. The goal for this phase is to strengthen your shoulder so it can handle everyday activities without pain.
Phase 4–Returning to Normal Activities (6+ Months)
The final phase of the rotator cuff repair protocol is all about getting back to your normal life. Whether you’re an athlete, a weekend warrior, or someone who just wants to lift groceries without pain, this phase helps you return to your usual activities.
Your doctor will guide you through sport-specific or activity-specific exercises to prepare your shoulder for the demands of your lifestyle. You’ll also learn how to prevent future injuries by maintaining good shoulder strength and flexibility.
