    What Is Tibial Tubercle Osteotomy?

Tibial tubercle osteotomy is a surgical procedure that corrects how the kneecap (patella) tracks during movement.

During the surgery, Dr. Brown will carefully cut the bump on the shin bone where the patellar tendon attaches and gently shift it to a better position. By shifting this attachment point to a more optimal location, the kneecap moves more smoothly along its natural groove, reducing patellar instability.

For patients with repeated instability, osteotomy knee surgery offers a long-term solution, often permanently fixing alignment issues. As with any surgery, there are risks, which Dr. Brown will discuss in detail with you at your consultation.

Who Should Get Tibial Tubercle Osteotomy?

This surgery is often recommended for patients who suffer from recurrent patellar dislocations or chronic instability, where their knee frequently gives way. While some issues can be managed with bracing or physical therapy, structural misalignment may require surgical correction. In many cases, surgery is necessary to prevent further cartilage damage.

Certain conditions make TTO particularly beneficial. These include patellar maltracking, a high-riding kneecap (patella alta), an abnormal TT-TG distance or early patellofemoral arthritis caused by uneven pressure on the joint.

Tibial Tubercle Osteotomy Surgery and Recovery

Dr. Brown will follow a carefully structured approach when conducting TTO surgery. First, he’ll use an arthroscope to access your knee. Then, the bone is cut and repositioned into a more stable alignment. Finally, it is secured using surgical screws to allow the bone to heal in its new position.

    Recovery is gradual:

    • The first six weeks: Use weight-bearing crutches and a brace while the bone heals in place.

    • Between three and six months: Begin physical therapy and guided strengthening to return to low-impact activities.
    • Six months and beyond: Increase to high-impact sports or demanding activity as recovery permits.

Physical therapy is crucial for a successful outcome, restoring strength, mobility and neuromuscular control.

Dr. Barrett Brown is a very fine physician. He is also a friend. While he is a surgeon he said this about my horribly painful shoulder. First we will try an injection performed by our specialist using ultra sound. (It was painless.) If that does not relieve your pain we will do a cat scan to evaluate the rotator cuff.

I have, unfortunately, been a frequent visitor to the Fondren Orthopedic clinic as I cannot seem to stay on my feet 😅. My most recent experience with Dr. Brown has been nothing short of excellent. From the initial exam to surgery and then the follow up and discussion of how I am healing have been extremely thorough and communication has been a 10. I would definitely recommend.
